Trap Botox has become one of the more talked about uses of Botox in recent years, partly because of the shoulder line it can create. It uses the same purified protein that softens frown lines and crow's feet, but places it in a much larger muscle: the trapezius, which runs from the base of the skull and the side of the neck out to the top of each shoulder.

How it works

The upper trapezius is the muscle you can feel when you shrug. When it is large or very active, it fills the slope between the neck and the shoulder, which can make the neck look shorter and the shoulders look high and square.

Botox relaxes the muscle by blocking the signal that tells it to contract. A muscle that works less gradually becomes smaller, in the same way that masseter Botox slims a wide jaw. As the trapezius slims, the curve from neck to shoulder becomes longer and more gently sloped. That is the look people often describe as Barbie Botox.

What it can and cannot do

That is why the assessment matters. A shoulder that looks bulky can be mostly muscle, mostly fat or a mix of both, and only the muscle responds to Botox.

Who it suits

The best results tend to come in people whose upper trapezius is visibly prominent, often because of training, posture or simply their natural build. People who want a softer, longer neckline for a particular look, or who feel their shoulders sit high, are the usual candidates.

It suits fewer people who depend on their shoulders for heavy lifting, overhead work or competitive sport, because the same relaxation that slims the muscle also reduces its strength for a while. Botox in the trapezius for appearance is also an off-label use of the product. That is common and accepted practice, but it is something your injector should explain before treatment.

The treatment

Trap Botox is an office procedure that usually takes around fifteen minutes. The muscle is assessed while you are relaxed and while you shrug, then a series of injections is placed into each side. Most people find it tolerable without numbing, and there is no downtime in the usual sense.

The aftercare is the same as for Botox elsewhere: avoid massaging the area, skip strenuous exercise for the rest of the day and keep heavy shoulder work light at first. The Botox aftercare guide covers the details.

Results and how long they last

The muscle begins to relax within about a week or two. The change in shape follows more slowly, because it depends on the muscle getting smaller, so most people judge the result over several weeks rather than days. The effect of the Botox itself typically lasts three to four months, and the slimmer contour can fade more gradually as the muscle rebuilds. The guide to how long Botox lasts explains what affects duration.

Safety and side effects

The most common effects are small bruises, tenderness and a dull ache at the injection points, which pass within days. Because the trapezius helps lift and steady the shoulders, some people notice weakness when carrying heavy bags, lifting weights or reaching overhead. Placement matters too: product that spreads to nearby muscles can affect how the neck and shoulder move. These effects are temporary, but they are the reason dose and placement should be decided by someone who knows the anatomy well.

Combining it with other treatments

Trap Botox is often part of a wider plan for the neck and jawline. It pairs with a Nefertiti lift, which uses Botox to soften neck bands and define the jaw, and with masseter Botox for a slimmer lower face. For lines across the front of the neck, see the guide to neck lines.

The short version

Trap Botox relaxes the upper trapezius so the muscle slims over several weeks, giving a longer looking neck and a softer shoulder line. The effect lasts around three to four months and needs repeating to maintain. It works when the bulk is muscle, not fat, and it is less suited to people who rely on shoulder strength. A careful assessment decides whether it will give you the shape you are after.

Curious whether trap Botox would change your shoulder line?

A consultation looks at the muscle, your posture and how you use your shoulders, so you know whether Botox will give you the shape you want.

Trap Botox is Botox injected into the upper trapezius, the muscle that slopes from the side of the neck to the top of the shoulder. Relaxing it lets the muscle slim over time, which can make the neck look longer and the shoulder line smoother. It is sometimes called Barbie Botox or traptox.
The muscle starts to relax within about a week or two. The slimming itself is gradual, because it comes from the muscle getting smaller with less use, so the full change in shape usually shows over several weeks.
Like Botox elsewhere, the effect usually lasts about three to four months before the muscle gradually returns to its usual activity. The slimmer shape can fade more slowly than that, as the muscle takes time to rebuild.
Some people say their upper shoulders feel less tight afterwards. It is mainly a cosmetic treatment, though, and ongoing pain deserves its own medical assessment rather than being treated as a side benefit.
Bruising, tenderness and a mild ache at the injection points are the most common. Some people notice their shoulders feel weaker when lifting heavy things or reaching overhead. These effects are temporary.
People who rely on upper body strength for work or sport may find the weakness gets in the way, and anyone pregnant, breastfeeding or with a neuromuscular condition should not have it. A consultation covers this.
